The Dementia Knowledge to Action Program: Rural and Remote Edition develops participants to become Dementia Guides, who can lead, coach, and influence within their care settings in rural and remote communities. This program includes face-to-face workshops as well as virtual classrooms. Participants are guided by a dedicated mentor to support planning and implementation of a care improvement project in the workplace.
To harness the best value from this program we ask that an organisational decision maker meets with the DTA Program intakes team. Program readiness as well as how to select the best candidate for the program will be explored.
To be eligible for this program, candidates must be nominated by their organisation. They need to be:
- Working frontline (or with staff working at the front line of care) in residential or community aged care or multi-purpose services in Australia.
- Knowledgeable about dementia.
- Have at least 2 years experience in supporting people living with dementia.
- Currently work in a rural or remote location, as defined by the 2023 Modified Monash Model geographical classification system. Eligible classifcations are MMM3 – MMM7. You can check your classification here.
Dementia Guides will identify an opportunity to enhance dementia care in their care setting.
Throughout the program they will be guided to plan, implement and complete a care improvement project.
A Certificate of Completion is awarded upon successful completion of the program, whereby participants will become certified Dementia Guides.

- The Dementia Guide’s Path from Knowledge to Action
- Guiding Dementia Care
- An Introduction to Care Improvement
- Understanding and Responding to Changed Behaviour
- Leading Care Improvement
Participants will receive:
- Mentor support
- Interactive digital learner workbook
- Online resource hub with tools and templates
- Communities of Practice to connect with experts and other Dementia Guides
