A New Initiative from Silver Chain, Responding to a Community’s Needs

Dementia is on the rise. For Australians, dementia is the second leading cause of death and its numbers are estimated to double over the next 30 years.

With lack of understanding, strain on carers, and rising costs in equipment and care needs, the World Health Organisation have declared dementia a ‘Public health priority’.

This presentation will look at how Silver Chain are adapting services to meet community needs, by introducing a new pilot project focusing on holistic palliative dementia care for those living at home.

  • Recognise the strain and personal impact caring for a loved one in palliative care, has on a family.
  • Learn about Silver Chain’s new Palliative Dementia Service and how it’s evolving around the complexities of individual cases.
  • Discover how the service helps to prepare and support families to consider Advance Care Planning, ease potential transitions to hospital, hospice and Residential Aged Care, and acknowledge loss while also celebrating the special times.
