About this Course
This course is designed for care partners, care coordinators, and case managers who support people living with dementia in the community. It focuses on person‑centred care planning and enablement approaches, including setting meaningful goals, applying practical strategies, using assistive technologies, and working with multidisciplinary services. Learners will build skills in monitoring change and reviewing care plans to support independence, safety, and wellbeing over time. Learning pathway: Learners who have limited knowledge and experience supporting people with dementia in community settings, may wish to complete Practical Dementia Care for Community Care Workers before undertaking this course.Learning Outcomes
After completing this course, staff should be able to:- Explain what enablement means in dementia care
- Align goals of care with what matters most to the person
- Apply enablement strategies: Cognitive, Functional, Emotional, Behavioural, Environmental
- Understand what to do when support workers observe decline or change
- Understand what, when and how assistive technology and devices can help
